Low food skills are barriers to healthy eating, while higher food preparation skills are associated with better diet quality including increased intake of vegetables and fruits; lower intake of calories, fat, and sugar; and lower intake of fast food, all of which are associated with reduced risk for diet-related chronic disease. Various programs and interventions have been developed to improve food skills; however, methodological limitations including non-experimental study designs, lack of follow-up data, and use of non-validated assessment tools highlight the need for more stringent study designs and development of validated measurement tools.
Survey Overview
Food Selection and Planning, Food Preparation, Food Safety and Storage
Measuring or quantifying food skills is challenging because there is no reference standard for comparison. Self-reported familiarity with cooking techniques was identified as a motivator for food preparation. Confidence in food preparation skills may influence purchasing behaviors and diet quality, and attitudes and beliefs affect the quantity, frequency, and time spent preparing food. Currently, validated surveys and scales measure only some aspects of food skills, such as food safety or self-reported technical cooking skills, and may not assess food skills as an overall construct. Others identified cooking skills as distinct from food skills. However, cooking skills would generally be identified as a component of food skills. Conversely, other surveys focus on nutrition literacy in the name of food literacy. Although nutrition literacy is important, it is only one aspect of food literacy.
Researchers at Brescia University College at Western University have developed a Food Skills Questionnaire (FSQ) a comprehensive tool for assessing food skills in three domains (Food Selection and Planning, Food Preparation, and Food Safety and Storage) in individuals or groups with basic to intermediate food skills. Researchers have also supplementary questions which can boost the assessment of food skills to an advanced level, although these haven’t been validated and can also provide a series of food-related behavioural and demographic questions which can be used in combination with the FSQ.
The FSQ shows strong potential for evaluating basic (e.g., peeling, chopping, slicing) to intermediate (e.g., meal planning) food skills in an effective and feasible manner. It can also capture changes in specific domains, allowing the development of more focused nutrition education and skills-based interventions. Ultimately, higher food skills in the population could result in increased intakes of vegetables and fruits and lower intakes of calories, fat, sugar, and fast food, potentially reducing the risk for chronic diseases.

WHEREAS Western owns all intellectual property rights to the copyright material called Food Skills Questionnaire (the “Licensed Material”).
AND WHEREAS the Licensee wishes to use the Licensed Material for the purpose of education and training (hereinafter the “Purpose”) and Western desires to license the Licensed Material to the Licensee.
NOW THEREFORE, the parties, for good and valuable consideration, the receipt and sufficiency of which is acknowledged, hereto agree as follows:
1. This License Agreement is a legally binding agreement between Licensee and The University of Western Ontario (“Western”) for the “Food Skills Questionnaire, Western Technology ID: W-21-030” and all associated documentation (together, the “Product”) developed by Dr. Paula Dworatzek, Dr. June Matthews, Dr. Jamie Seabrook, Ms. Lauren Kennedy and Ms. Emily Kichler (collectively “Inventors”) at Brescia University College at the University of Western Ontario. Licensee use of the Licensed Material is subject to the terms and conditions set forth below.
2. LICENSE TO USE:
Western hereby grants to the Licensee a non-exclusive, revocable, non-transferable, limited license to use the Product solely on the terms, conditions and restrictions contained in this Agreement. The rights granted to the Licensee shall, mean the right to use the Product for the limited purposes of assessing and evaluating the Food Handling Skills of participants within Licensee’s organization or Licensee’s business
3. LICENSE FEE:
In consideration for the rights granted to the Licensee by Western under this Agreement, Licensee must pay to Western the discounted License fee as listed on the Product website and dependent on the number of users Product is purchased for. The License Fee shall be due and payable upon acceptance of the terms of this Agreement. Neither all nor any portion of the License Fee shall be refundable to the Licensee under any circumstances.
4. DELIVERY OF PRODUCT:
Western will provide the Licensee with the Product via web delivery after the Licensee has executed the Agreement and Western has received payment of the License Fee from the Licensee.
5. OWNERSHIP & RESTRICTIONS:
Licensee will have no ownership rights in the Product. Rather, Licensee will have a limited license to use the Product as long as this License Agreement remains in effect. The Product and any and all knowledge, know-how and/or techniques relating to the Product, in whole or in part, is and will remain the sole and absolute property of Western and Western owns any and all right, title and interest in and to the Product.
This Product has been prepared initially in the English language, and the English version is the only authorized version of the Product. Any translated version is not endorsed or authorized by Western as an official copy of the Product. Any translation of the Product shall require the express and prior consent of Western (direct all enquiries to ipm@uwo.ca). Should permission be granted to translate the Product, Western does not represent or warrant that any translated versions of the Product are scientifically valid or appropriate for the use intended.
6. ACKNOWLEDGEMENT OF WESTERN:
If Licensee uses or references the Product in any publication (including scientific publications, electronic documents or websites) or derivative work, Licensee must seek prior approval and include appropriate acknowledgment of Western and Inventors of the Product.
7. DISCLAIMER OF REPRESENTATIONS AND WARRANTIES:
NOTHING IN THIS AGREEMENT IS OR WILL BE CONSTRUED AS A REPRESENTATION OR WARRANTY BY WESTERN AS TO THE FITNESS OR MERCHANTABILITY OF THE PRODUCT OR WITH RESPECT TO ANY CLAIMS BY THIRD PARTIES FOR INFRINGEMENT. AS OF THE EFFECTIVE DATE OF THIS AGREEEMENT, NO THIRD PARTY HAS LEVIED ANY INFRINGEMENT CLAIM AGAINST WESTERN AND LICENSEE ACCEPTS AND ASSUMES THE RISK OF UTILIZING THE PRODUCT AS ENVISAGED AND AGREED ABOVE IN THIS AGREEMENT.
8. LIMITATIONS OF LIABILITY:
WESTERN’S LIABILITY FOR DAMAGES TO LICENSEE UNDER THIS AGREEMENT SHALL BE LIMITED TO THE AMOUNTS ACTUALLY PAID BY THE LICENSEE TO WESTERN IN THE CALENDAR YEAR IN WHICH THE PERTINENT CAUSE OR CAUSES OF ACTION AROSE. IN NO EVENT SHALL WESTERN BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, OR CONSEQUENTIAL DAMAGES, INCLUDING LOSS OF USE, LOSS OF PROFITS OR INTERRUPTION OF BUSINESS, HOWEVER CAUSED OR ON ANY THEORY OF LIABILITY.
9. INDEMNITY:
The Licensee agrees to indemnify and hold Western harmless from and against any loss, liability, cost, expense or damage (including reasonable legal fees) arising directly or indirectly out of any claim, suit, action or judgment relating to the Licensed Product, including but not limited to the provision by the Licensee of any products or services, any representations, warranties or covenants made by the Licensee, any failure by the Licensee to comply with any of the terms of this Agreement, or violation by the Licensee of any laws, regulations or any third party rights.
10. TERM:
This Agreement commences on the date the Product is electronically or physically delivered to the Licensee and continues in effect for 3 years from the delivery date. Licensee may terminate this Agreement at any time by ceasing use of the Product and providing written notice to such effect to Western. Western may terminate this Agreement upon giving LICENSEE 30 days' notice. If Licensee finds itself, upon the provision of the 30 days’ termination notice, in the midst of a research ethics board approved study or trial which utilizes the Product, Licensee may conclude the official study or trial and then must immediately, cease use of the Product. Notwithstanding, this Agreement will terminate immediately upon written notice from Western if Licensee fails to comply with any provision of this Agreement. On termination Licensee must immediately delete and destroy all electronic and physical copies of the Product in Licensee possession or control, but for any that would be required to be maintained by law, a research ethics protocol or board. On any termination of this Agreement, the Disclaimer of Representations and Warranties, Limitation of Liability and Indemnity provisions of this Agreement shall survive, notwithstanding such termination.
11. GENERAL:
Governing Law. This Agreement is governed by the laws of the Province of Ontario, Canada and each of the parties hereto irrevocably attorns to the exclusive jurisdiction of the courts of the Province of Ontario without regard to conflicts of laws principles. The parties agree that the United Nations Convention on Contracts for the International Sale of Goods does not apply to this Agreement.
Successors and Assigns. These terms and conditions inure to the benefit of and shall be binding upon, the parties’ respective heirs, successors or permitted assigns. The Licensee shall not assign this Agreement.
Invalidity of Terms. If any of these terms and conditions is found by a court of competent jurisdiction to be invalid, the parties hereto nevertheless agree that the court should endeavour to give effect to the parties’ intentions as reflected in such term or condition, and the other terms and conditions shall remain in full force and effect.
Entire Agreement: This Agreement constitutes the entire agreement between the parties pertaining to the subject matter hereof and supersedes all agreements, understandings, negotiations and discussions, whether oral or written, of the parties and there are no warranties, representations or other agreements between the parties in connection with the subject matter hereof except as specifically set forth herein.
Survival: The Licensee’s obligations under this Agreement, Indemnity and the Limitations of Liability Provision shall survive the termination of the Agreement.

This License Agreement is a legally binding agreement between Licensee and The University of Western Ontario (“Western”) for the “Food Skills Questionnaire, Western Technology ID: W-21-030” and all associated documentation (together, the “Product”) developed by Dr. Paula Dworatzek, Dr. June Matthews, Dr. Jamie Seabrook, Ms. Lauren Kennedy and Ms. Emily Kichler (collectively “Inventors”) at Brescia University College at the University of Western Ontario. Licensee use of the Licensed Material is subject to the terms and conditions set forth below.
2. LICENSE TO USE:
Western hereby grants to the Licensee a non-exclusive, revocable, non-transferable, limited license to use the Product solely on the terms, conditions and restrictions contained in this Agreement. The rights granted to the Licensee shall, mean the right to use the Product for the limited purposes of assessing and evaluating the Food Handling Skills of participants within Licensee’s organization or Licensee’s business
3. LICENSE FEE:
In consideration for the rights granted to the Licensee by Western under this Agreement, Licensee must pay to Western the discounted License fee as listed on the Product website and dependent on the number of users Product is purchased for. The License Fee shall be due and payable upon acceptance of the terms of this Agreement. Neither all nor any portion of the License Fee shall be refundable to the Licensee under any circumstances.
4. DELIVERY OF PRODUCT:
Western will provide the Licensee with the Product via web delivery after the Licensee has executed the Agreement and Western has received payment of the License Fee from the Licensee.
5. OWNERSHIP & RESTRICTIONS:
Licensee will have no ownership rights in the Product. Rather, Licensee will have a limited license to use the Product as long as this License Agreement remains in effect. The Product and any and all knowledge, know-how and/or techniques relating to the Product, in whole or in part, is and will remain the sole and absolute property of Western and Western owns any and all right, title and interest in and to the Product.
This Product has been prepared initially in the English language, and the English version is the only authorized version of the Product. Any translated version is not endorsed or authorized by Western as an official copy of the Product. Any translation of the Product shall require the express and prior consent of Western (direct all enquiries to ipm@uwo.ca). Should permission be granted to translate the Product, Western does not represent or warrant that any translated versions of the Product are scientifically valid or appropriate for the use intended.
6. ACKNOWLEDGEMENT OF WESTERN:
If Licensee uses or references the Product in any publication (including scientific publications, electronic documents or websites) or derivative work, Licensee must seek prior approval and include appropriate acknowledgment of Western and Inventors of the Product.
7. DISCLAIMER OF REPRESENTATIONS AND WARRANTIES:
NOTHING IN THIS AGREEMENT IS OR WILL BE CONSTRUED AS A REPRESENTATION OR WARRANTY BY WESTERN AS TO THE FITNESS OR MERCHANTABILITY OF THE PRODUCT OR WITH RESPECT TO ANY CLAIMS BY THIRD PARTIES FOR INFRINGEMENT. AS OF THE EFFECTIVE DATE OF THIS AGREEEMENT, NO THIRD PARTY HAS LEVIED ANY INFRINGEMENT CLAIM AGAINST WESTERN AND LICENSEE ACCEPTS AND ASSUMES THE RISK OF UTILIZING THE PRODUCT AS ENVISAGED AND AGREED ABOVE IN THIS AGREEMENT.
8. LIMITATIONS OF LIABILITY:
WESTERN’S LIABILITY FOR DAMAGES TO LICENSEE UNDER THIS AGREEMENT SHALL BE LIMITED TO THE AMOUNTS ACTUALLY PAID BY THE LICENSEE TO WESTERN IN THE CALENDAR YEAR IN WHICH THE PERTINENT CAUSE OR CAUSES OF ACTION AROSE. IN NO EVENT SHALL WESTERN BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, OR CONSEQUENTIAL DAMAGES, INCLUDING LOSS OF USE, LOSS OF PROFITS OR INTERRUPTION OF BUSINESS, HOWEVER CAUSED OR ON ANY THEORY OF LIABILITY.
9. INDEMNITY:
The Licensee agrees to indemnify and hold Western harmless from and against any loss, liability, cost, expense or damage (including reasonable legal fees) arising directly or indirectly out of any claim, suit, action or judgment relating to the Licensed Product, including but not limited to the provision by the Licensee of any products or services, any representations, warranties or covenants made by the Licensee, any failure by the Licensee to comply with any of the terms of this Agreement, or violation by the Licensee of any laws, regulations or any third party rights.
10. TERM:
This Agreement commences on the date the Product is electronically or physically delivered to the Licensee and continues in effect for 3 years from the delivery date. Licensee may terminate this Agreement at any time by ceasing use of the Product and providing written notice to such effect to Western. Western may terminate this Agreement upon giving LICENSEE 30 days' notice. If Licensee finds itself, upon the provision of the 30 days’ termination notice, in the midst of a research ethics board approved study or trial which utilizes the Product, Licensee may conclude the official study or trial and then must immediately, cease use of the Product. Notwithstanding, this Agreement will terminate immediately upon written notice from Western if Licensee fails to comply with any provision of this Agreement. On termination Licensee must immediately delete and destroy all electronic and physical copies of the Product in Licensee possession or control, but for any that would be required to be maintained by law, a research ethics protocol or board. On any termination of this Agreement, the Disclaimer of Representations and Warranties, Limitation of Liability and Indemnity provisions of this Agreement shall survive, notwithstanding such termination.
11. GENERAL:
Governing Law. This Agreement is governed by the laws of the Province of Ontario, Canada and each of the parties hereto irrevocably attorns to the exclusive jurisdiction of the courts of the Province of Ontario without regard to conflicts of laws principles. The parties agree that the United Nations Convention on Contracts for the International Sale of Goods does not apply to this Agreement.
Successors and Assigns. These terms and conditions inure to the benefit of and shall be binding upon, the parties’ respective heirs, successors or permitted assigns. The Licensee shall not assign this Agreement.
Invalidity of Terms. If any of these terms and conditions is found by a court of competent jurisdiction to be invalid, the parties hereto nevertheless agree that the court should endeavour to give effect to the parties’ intentions as reflected in such term or condition, and the other terms and conditions shall remain in full force and effect.
Entire Agreement: This Agreement constitutes the entire agreement between the parties pertaining to the subject matter hereof and supersedes all agreements, understandings, negotiations and discussions, whether oral or written, of the parties and there are no warranties, representations or other agreements between the parties in connection with the subject matter hereof except as specifically set forth herein.
Survival: The Licensee’s obligations under this Agreement, Indemnity and the Limitations of Liability Provision shall survive the termination of the Agreement.

WHEREAS Western owns all intellectual property rights to the copyright material called Food Skills Questionnaire (the “Licensed Material”).
AND WHEREAS the Licensee wishes to use the Licensed Material for the purpose of education and training (hereinafter the “Purpose”) and Western desires to license the Licensed Material to the Licensee.
NOW THEREFORE, the parties, for good and valuable consideration, the receipt and sufficiency of which is acknowledged, hereto agree as follows:
